Grades 6-8
If you are seeking full-time enrollment with Utah Online, please submit the following documents:
- Proof of Residency (a bill or anything that shows you live where you live.)
- Birth Certificate.
- Immunizations or Health Department Exemption.
- Utah Online Exemption Form (Please click to download.)
- Special Education Papers if applicable (IEP and Eligibility, 504 Plan.)
To officially enroll in our school, you must submit ALL required documents.
Please submit ONLY when you have obtained all 4 documents listed above (5 if special education).

Grades 9-12
If you are seeking full-time enrollment with Utah Online, please submit the following documents:
- Proof of Residency (any document that indicates name and Utah address.)
- Birth Certificate.
- Immunizations or Health Department Exemption.
- Transcript (if applicable – only for grades 9-12 or if you have previous high school credit.)
- Utah Online Exemption Form (Please click to download.)
- Special Education Papers if applicable (IEP and Eligibility, 504 Plan.)
The Online Exemption Form ensures that parents and students understand that all graduation requirements must be met in order for students to earn a high school diploma with Utah Online.
To officially enroll in our school, you must submit ALL required documents.
