Step 1: Register Online

  • Complete the UOS application form HERE.

Step 2: Upload Documents
After the initial application, you will be emailed a link to upload documents and finish your application.  The following paperwork is required to be uploaded into the enrollment portal to finalize registration.

  • Proof of Residence (such as city utility bill)
  • Official Birth Certificate
  • Current Immunizations (or exemption form)
  • Special Education Papers if applicable (IEP, 504 Plan)

Step 3: Prepare for placement

  • Watch the placement video online. (Link coming soon.)
  • View course offerings. A list of available courses can be found HERE.
  • You will also need to read the Utah Online School Handbook.

Step 4: Select Courses

  • A counselor or teacher will contact you to help you make your course selections and finalize your enrollment.
