After the initial application, you will be emailed a link to finalize enrollment and upload documents. All necessary documents (copies of Birth Certificate, Current Immunization or Exemption Form, and Proof of Residency) will need to be uploaded into the enrollment portal to be considered for enrollment in Utah Online School.
